Harnessing the power of community with virtual health coaching groups, HealCommunity is supporting patients across the US to manage and reverse chronic conditions through lifestyle medicine and behavior change.

As behavior change experts, we have the curated content, lifestyle protocols, community support and access to 24/7 resources that will support patients to new levels of sustainable health. Led by Board-Certified health coaches, every patient gets face-to-face time with their coaching team and their group throughout the 6-month foundation program.

We are an extension of your care team and work closely with your provider to provide 360° of support as you begin to build new habits with confidence and community.

This tech platform is an enhancement to the coaching program you’ll enter to help you set new habits, access digital resources, chat with your group and coaching team, and join all of your live coaching sessions.

Inside this app, you’ll find:

- Your 24 week program content for easy access
- A habit tracker where you will work with your coach to begin setting new healthy habits
- A chat room to talk with your coaching team and your group
- A calendar to let you know when your live sessions will be and how to join them

MEMBER DASHBOARD

Your dashboard will provide a summary of the your program, show you what pillar you are currently in, share the schedule of live sessions, access to group chat, content, activities and your habit tracker.

TRACK YOUR GROUP ACTIVITIES

You can select the weekly activities offered by the app, and an opportunity to download the workbook. You will also find a way to learn more on the different pillars in this section.

ADD AND TRACK YOUR HABITS

You can add habits that you build in consultation with your coach and provider, and the app can remind you in the morning, afternoon or evening so you won’t forget. New tiny habits are the key to big health changes!

YOUR CALENDAR

You will access a monthly calendar to show you your daily activities and upcoming live sessions with your group

CHAT WITH YOUR COACHES OR GROUP

Your coaching team will be in consistent contact with you and the group to share information, discuss topics further, check in you and your group, and be part of your journey as often as you’d like!
